Surfaces contaminées aux antinéoplasiques/Surface contamination with antineoplastic drugs

Nous avons complété notre plus grande étude multicentrique de surveillance environnementale! En 2017, 83 hôpitaux à travers le Canada ont participé à notre étude. 953 échantillons ont été analysés pour la présence de 10 antinéoplasiques : cyclophosphamide, ifosfamide, méthotrexate, cytarabine, gemcitabine, 5-fluorouracile, irinotécan, docétaxel, paclitaxel et vinorelbine. Autre nouveauté cette année, les participants avaient accès à leurs résultats à partir d’un site web leur permettant de consulter leurs résultats historiques ainsi que de se comparer au reste du Canada. Il n’existe pas de limite sécuritaire d’exposition aux médicaments antinéoplasiques pour les travailleurs; nous permettons à nos centres participants d’identifier quels sites de prélèvement sont à cibler (c’est-à-dire ceux dont les concentrations mesurées étaient supérieures au 75e ou au 90e percentile mesuré à partir des résultats canadiens).

Les résultats ont été présentés au congrès Midyear Clinical Meeting de l’ASHP et il nous fait plaisir de partager l’affiche qui peut être consultée ici. Nous constatons que la majorité des surfaces demeurent contaminées au cyclophosphamide, mais que la concentration mesurée est basse et diminue au fil des années. Les surfaces les plus contaminées demeurent la grille de la hotte, le plancher devant la hotte et les bras des fauteuils où les patients reçoivent de la chimiothérapie.

We have completed our largest environmental surveillance study to date! In 2017, 83 hospitals across Canada participated. 953 samples were analyzed for the presence of 10 antineoplastic drugs: cyclophosphamide, ifosfamide, methotrexate, cytarabine, gemcitabine, 5-fluorouracile, irinotecan, docétaxel, paclitaxel and vinorelbine. New this year: participants can access their results through a website that allows them to see their historical results and also to compare with the rest of Canada. There is no safe exposure limit for antineoplastic drugs; our study helps participating centers identify which sampling site should be targeted (those with concentrations measured above the 75th or 90th percentile that was calculated from Canadian results).

The results were presented at the ASHP Midyear Clinical Meeting and we are happy to share the poster that can be consulted here. The majority of surfaces were contaminated with cyclophosphamide, but the concentration measured was low and is reduced over the years. The surfaces that were the most contaminated were the front grille of the hood, the floor in front of the hood and the arm rests of the chairs were chemotherapy is administered.
